PARENT/CAREGIVER AGREEMENT
I wish to enrol my child in the Karori West OSCAR Group. I acknowledge and agree to the following conditions:
1) My payment of fees will be in accordance with the terms specified in the current Schedule of Fees.
2) All children are to be collected no later than 5.45pm each day. I understand that I will incur a late fee of $15 for each fifteen minute period I am late after 5.45pm.
3) Only persons identified on my enrolment form shall be allowed to pick up my child, unless I have previously advised the Supervisor.
4) I agree to inform the Supervisor in advance of each session that my child will be absent from, by leaving a message on the KWOSCAR answerphone (04 476 6400).
5) I will not send my child to KWOSCAR if s/he is unwell. The Group does not have the facilities to care for sick children.
6) The Supervisor must be advised immediately of any change in contact details, address, care arrangements, or other important details relating to my chid.
7) The Supervisor may arrange urgent medical treatment that is required, at my expense.
8) I agree to advise the Supervisor of any situation that may be disturbing my child.
9) It is my responsibility to ensure that my child behaves in an appropriate manner while at KWOSCAR. Behaviour that consistently affects the quality of care available to other children, or construes serious misconduct, may result in removal from the programme.
10) I agree to pay for any property that is wilfully or recklessly damaged by my child.
11) I understand that my child will be responsible for their own property.
12) Parents/caregivers will be informed before any children are taken on outings, and I will be asked to complete a permission slip.
13) I give permission for photographs of my child to be taken for genuine resources and publicity purposes.
14 ) I agree to abide by all rules and procedures of the Group. I confirm that I have read a copy of the Standards, Procedures and Policies document and have discussed these with my child.
15 ) I understand that all staff are screened, trained and have appropriate experience in the job they do. I understand that the Group complies with Health and Safety policies and the National OSCAR Association standard. I understand that accidents will happen and I will not hold the Group responsible for genuine mishaps.
16) While the management of KWOSCAR Group Inc, and its staff, take care to proper supervision of all children, neither the management or staff shall have any personal liability in respect of any act or omission arising out of any session or activity of the programme.
17) I am aware of, and understand, the Group’s complaints procedure.
18) The Group is an incorporated society made up of parents/caregivers of attending children. All parents/caregivers are expected to be involved in the running of the Group, including holding Committee positions as they become available. All parents/caregivers are welcome to attend Group meetings.
19) Notice period: cessation of fulltime membership requires four weeks’ notice in advance, in writing.
Changes to, or cessation of, part-time membership can only be made at the end of term.
20) I acknowledge that failure to comply with any of the conditions of this Agreement may, at the Committee’s discretion, result in suspension or termination of membership to the KWOSCAR Group.
